A man and two children died in a head-on collision on Williamson Valley Road Saturday. The Yavapai County Sheriff’s Office reports Chino Valley resident Jesse Forman was driving a Toyota 4Runner with his 3 young children and the mother of the children when it crossed the double lines and collided with a Jeep SUV.
Forman and one child were pronounced dead at the scene. A second child died while being transported to the hospital. Three adults were airlifted to hospitals for treatment, and the third child is listed in critical condition at a hospital in Phoenix.
The Sheriff’s Office is investigating the cause of this crash.
